Acknowledgment & Consent:

  • I understand that massage therapy provided is for relaxation and therapeutic purposes only.
  • I have informed the therapist of all known medical conditions, allergies, injuries, or surgeries.
  • I understand massage therapists do not diagnose or prescribe.
  • I acknowledge massage may involve light to deep pressure and will communicate discomfort.
  • I consent to receive massage therapy services.
  • I understand draping will be used and inappropriate behavior ends session immediately.

Liability Waiver:

I release Royal Revitalization Spa, its owners, employees, and contractors from all liability concerning any injury that may occur during or after my massage session. I affirm that I have stated all known medical conditions and will keep the therapist updated on my health.

Cancellation & Late Policy:

understand a 24-hour notice is required to cancel or reschedule. Missed appointments or cancellations with less than 24 hours’ notice may result in a fee up to the full service amount.
